Job Description Job Description Salary: Position Summary The
Hospice Nurse Practitioner (NP) provides advanced clinical care to
hospice patients across settings, including private homes, assisted
living facilities, and skilled nursing facilities. This role works
collaboratively with the hospice interdisciplinary group (IDG) to
ensure high-quality, patient-centered, and compliant end-of-life
care. The NP supports symptom management, patient and family
education, advance care planning, and regulatory documentation in
alignment with CMS and hospice Conditions of Participation. Key
Responsibilities Conduct face-to-face encounters for hospice
eligibility, recertification, and symptom management Perform
comprehensive patient assessments and develop individualized plans
of care Manage pain and symptoms using evidence-based palliative
care practices Collaborate with physicians, nurses, social workers,
chaplains, and other IDG members Provide education and guidance to
patients, families, and caregivers regarding disease progression
and comfort measures Document all clinical encounters accurately
and timely in the electronic medical record (EMR) Participate in
IDG meetings, quality improvement initiatives, and clinical case
reviews as needed Ensure compliance with CMS, state regulations,
and agency policies Required Qualifications Active Nurse
Practitioner license in the state of Texas National NP
certification (ANCC or AANP) Active DEA license and prescriptive
authority Minimum of 1 year experience as a Nurse Practitioner
(hospice, palliative care, or geriatrics preferred) Strong
knowledge of hospice philosophy, end-of-life care, and symptom
management Excellent clinical judgment, communication, and
documentation skills Valid drivers license, reliable
transportation, and current auto insurance
