Job Description:

Encompass Health is seeking a dedicated LPN / LVN to join our team, where you will play a pivotal role in delivering exceptional healthcare services in both facility-based and home-based settings. Our focus is on comprehensive rehabilitation and post-acute services tailored to meet the individual needs of our patients. This role demands a compassionate professional keen on providing the highest level of personalized patient care, enhancing patient recovery, and ensuring patient satisfaction through dedicated nursing practices. Provide nursing care compliant with established standards and practices. Administer medications and document therapeutic effects. Take vital signs and record patient progress. Collaborate with a multidisciplinary team to create and evaluate customized care plans. Educate patients and families on treatment plans and health maintenance. Assist with patient rehabilitation and recovery. Proficient in nursing best practices and medical protocols. Strong communication and interpersonal skills. Ability to work independently and with multidisciplinary teams. Capacity to make swift decisions in emergency situations. Medical Insurance Dental Insurance Vision Insurance 401k Current LPN/LVN license in the state of practice. CPR certification is required.
