Job Description:

as a Licensed Vocational Nurse (LVN) working with low acuity
patients in their homes. This position offers a compassionate
professional the opportunity to deliver personalized nursing care
in a one-on-one setting during the day, ensuring the highest level
of comfort and quality of life for our patients. As a crucial
member of our nursing team, you will utilize your skills in a
supportive and caring environment, directly contributing to the
lives of our patients and their families. Provide daily nursing
care as per the patient’s care plan. Monitor patient health status
and report changes to the assigned RN or supervisor. Administer
medications and complete treatments as prescribed. Educate patients
and families on the management of their condition and
post-treatment home care needs. Document all patient interactions
and care provided with accuracy and clarity. Collaborate with
multidisciplinary healthcare teams to assess patient needs and plan
continuing care. Excellent bedside manner and patient-centric
approach. Proficient in administering medication and using medical
equipment. Strong observational skills and attention to detail.
Effective communication and interpersonal skills. Ability to manage
stress and react effectively in emergency situations. Medical
Insurance Dental Insurance Vision Insurance Life Insurance Current
LVN license in the state of practice. Valid CPR certification.
Driver’s license and dependable transportation.
